I'm currently thinking about some missing menu items:

a.) Text Zoom
View -> Text Zoom -> 100% (also toolbar item with icon "ZOOM_100")
View -> Text Zoom -> Smaller (also toolbar item with icon "ZOOM_OUT")
View -> Text Zoom -> Larger (also toolbar item with icon "ZOOM_IN")

In Safari (the Mac OS X Web browser), there are "Make Text Bigger" and "Make Text Smaller" menu items under "View".

On the other hand, Preview (the Mac OS X PDF viewer) has "Zoom In" and "Zoom Out".

Perhaps they figured Preview's more likely to be used by publishing professionals, but Safari's more likely to be used by your grandmother?

Got this layout idea from Mozilla.

But both the gnome and kde styleguides suggesting: "View->Zoom In" (Ctrl+PLUS) and "View->Zoom Out" (Ctrl+Minus), so we might do it that way.

Gnome is using "View->Normal Size", KDE is using "View->Zoom..." without further explanation, so I tend to follow the gnome guideline here.
