    The answer to
    
    // XXX Should we call maxmind_db_lookup_process first?
    
    in maxmind_db.c is "yes", since it's possible to fill up our pipe
    between host name lookup intervals, at least on Windows. Note that we
    might want to move request processing to a thread.
