The attached patch adds dissectors for RFC 5285 RTP Header extensions.

It also adds two dissector tables, one for all RTP header extensions by
extension id (instead of by payload ID like the current one). And one for RFC
5285 header extensions (by id also).

RFC 5285 hdrext are normally refereed to by URI in the SDP negotiation (and the
numeric ID is agreed on there). But doing that would require adding the SDP
parsing bits and I'm too lazy (also I'm doing XMPP right now, maybe when I add
SIP support to my client).
