I've put together an updated patch with the following features:

* The main window title is constructed as:

  “window_name  <[user_specified_title]>  <[Wireshark_version]>”

  Where window_name is the filename or other title. This conforms to the  
  Windows, GNOME and KDE UI guidelines that you mentioned and ensures that
  WS icons in the task bar begin with the filename rather than the version. 

* Updates to the 'Welcome screen and title shows version' and 
  Layout > 'Custom window title' prefs in the Preferences dialog are displayed 
  immediately upon clicking OK, Apply, or Save.
