finalised sFlow v5 dissector

Please see attachment 3123 for dissector capability overview.

This dissector has COMPLETE sFlow v5 support, plus some additional record
formats support that were requested by InMon Corp. It has been fuzz tested by
myself, and also tested by InMon's engineers.

The patch is to the sFlow dissector in Wireshark 1.0.8 public release.
