Small patch to fix some digit dissection

(packet.dll version 4.0.0.1040), based on libpcap version 0.9.5, GnuTLS 2.6.3,

The digits dissection for BCD/IA5 with unknown numbering plan
was not decoding the number of digits field.
There was a bitmask error for hf_ansi_map_reservedBitHG and
an incorrect filter field name for hf_ansi_map_reservedBitD.
