SI3 and 4 Rest Octets IE decoding + misc fixes

1.4.1, with MIT Kerberos, with PortAudio V19-devel (built Oct 27 2008), with
AirPcap.

Running on Windows XP Service Pack 3, build 2600, with WinPcap version 4.0.2
(packet.dll version 4.0.0.1040), based on libpcap version 0.9.5, without
AirPcap.

Built using Microsoft Visual C++ 9.0 build 30729

The attached patch adds the decoding of SI3 and SI4 Rest octets IE according to
3GPP 44.018 release 7.14.0.
It also fixes a bug in de_rr_si2quater_rest_oct() function (uninitialized use
of pointer subtree2), and uses proto_tree_add_bits_item() in SI1, 2ter and
2quater Rest Octets IE.
