There are other reasons that could have caused this.

1) If you (someone) has a laptop connected to both your wired and
wireless vlans, and you turned on bridging between the two NICs on the
laptop.
2) If you have a loop on your network, but you forgot to enable spanning
tree on every port of every ethernet switch.
3) A bug in the HP ethernet switch code.
4) You could be under attack from some device which is using spoofed IP
addresses or spoofed MAC addresses.
