I found my problem with the MGCP plugin on Win32.
The 'plugin_reg_handoff' symbol wasn't being exported
via DLLEXPORT. Once that was added, the mgcp plugin
is found and correctly initialized.

I'll provide an updated Win32 binary distribution tomorrow.
