The informal policy so far has been to accept diffs via the e-mail
list at first, and once the developers see that a new developer is
submitting various high-quality changes, if the developer wants it, we
give him/her CVS access.

The CVS repository is on Gerald's machine; he's the one with the ultimate
control, but I'm the one who usually says "Gerald, how 'bout giving
so-and-so CVS access?".
